If your credit card has been lost, canceled or replaced, we will need a letter from the bank stating this.
You can upload the letter via your MyAccount area (click here to learn how).
Please make sure the letter contains the following information:
1. The Credit Card owner's name
2. The date of cancellation
3. The first and last 4 digits of the old card's* number
Please note: If your old card was replaced with a new one, please make sure to have the first 4 and last 4 digits of the new card as well as the old one.
