• Proof of Identification: (Passport, Local ID card, Driving License) – For each of the account holders.
• Proof of Address: Gas bill, Electric bill, Telephone bill, Current lease agreement, CRA Notice of assessment, GST Refund Letter) with your name, current address and dated within the last 3 months. – For each of the account holders.
• W-8BEN form - Please download, fill and upload this form into your My Account. – For each of the account holders.
• Financial Statement: a copy of an Original Bank, Loan/Mortgage or Credit Card account statement dated within the last 3 months. – For each of the account holders.
• Trusted Contact Person form -Please download, fill and upload this form into your My Account – For each of the account holders.
• LPOA – Please download, fill and upload this form into your My Account – For each of the account holders.
